  • The care plan outlines the type of care you need, the frequency of visits, and the specific tasks our care assistants will assist you with.

 

4. Discussion and Agreement:

  • We review the care plan with you and your family to ensure it meets your expectations and addresses all your needs.

  • Once we have your approval, we will finalise the plan.

 

5. Ongoing Monitoring:

  • Our team regularly monitors the quality of care you receive, making adjustments to the care plan as needed.
